He was predeceased by: his parents, John Kozak and Betty Kozak (Gunsallus); and his siblings, Randy and Lura Hockens (Daniel "Boone"). He is survived by: his wife Lynne Kozak of Osage, IA; his sons, Anthony "Tony" Kozak (Shelly) of Peosta, IA and Eric Kozak (Rhonda Akkerman) of Rose Creek, MN; his stepdaughters, Lynette Clayton (Rick) of Spearfish, SD and DeeAnn Midlang (Louis Lorenson) of Northwood, IA; his grandchildren, Zachary Kozak, Jacob Kozak, Shelby Kozak, Katy Olander (Stephen), Tatum Midlang and Sydney Midlang; and his siblings, Ann Aulwes-Allison (Bob), Ellen Novak (Wes), Ron Kozak (Marcia), Roger Kozak, Robert Kozak (Cheryl), Russell Kozak (Marianne) and Rex Kozak (Bonnie). He is also survived by several nieces and nephews.
