He was predeceased by: his father Blair; his mother-in-law Karla Lewis; his grandparents, Stan Thompson, Arden Liljedahl, Irvin Madden and Beth Madden; and his aunts and uncles, Nancy, Marvin Hickman, Calvin Madden and Cindy Madden.
He is survived by: his children, Porschea, Aspen, Victoria and Lincoln; his fiancée Hattie Lewis; his mother Sandra Madden of Northwood; his brother Derek Madden of Mason City; his grandmother Carolyn Thompson; and his aunts and uncles, Ron Liljedahl (Lori), Sharyl Anderson (Steve), Susan Odden, Jackie Berg (John), Nicole Ochoa (Israel) and Gail Schorg. He is also survived by numerous extended family and friends.
In lieu of flowers, memorials may be directed in care of Cody's family.
