He was predeceased by: his parents, John Middlekauff and Opal Middlekauff (Shaull); and his brother Norman Middlekauff. He is survived by: his former wife Viola Hodgson; his daughters, Crystal Fetter of North English, Linda Worrall (Harvey Gesling) of Cedar Rapids and Connie McMillan (Tim) of Parnell; and his sisters, Janice Garringer (Chuck) of Conroy and Joan Strasser (Larry) of South English. He is also survived by 10 grandchildren; 17 great grandchildren; one great great grandson.
A memorial fund has been established for Essence of Life Hospice.
