He was predeceased by: his parents, Donald Follett and Colleen Follett (Carroll); his father-in-law Maynard Finch; and his siblings-in-law, Mary Follett and Kevin Finch.

He is survived by: his wife Leslie Ann Finch; his son Aaron Follett of Newton; his siblings, Doug Follett of Jefferson, Jon Follett of Webster City, Janet Follett of Mesa, AZ and Dawn Holdgrafer (Keith) of Webster City; his mother-in-law Charlotte Finch of Webster City; and his sisters-in-law, Lori Flaws (Jim) of Carlisle and Becky Finch of Webster City. He is also survived by several nieces, nephews.

In lieu of flowers, memorials may be left to the family to contribute to organizations that were dear to Jeff.
