He was predeceased by: his parents, Gail Rusk and Eva Rusk (Bierma); his wife Mary Lee; and his siblings, Deloris Lang, Claude, George, Carl and Dale Rusk.
He is survived by: his children, Steve, Linda DiGiacomo (Mike), Deborah Cox (Kevin) and Cheryl Wedemeyer (Lyle); his brother Jim Rusk; his grandchildren, Michelle Parise (Jon), Elizabeth Christopher (Thomas), Amelia, Olivia Wedemeyer and James Forrest Cox; and his great grandchildren, Michael, Christopher, Cora Lee and Thomas.
In lieu of flowers, and as an expression of sympathy, memorials may be designated to Forrest's family.
