He was predeceased by: his parents, Donald Dean and Donna May Black (Ballenger); his wife Ruth Wade; and his brother Dennis. He is survived by: his sister Darla Lane of Higginsville, Missouri; and his stepdaughters, Gerianne Reichert (Mitch), Paula Niaves of Muscatine and Julie Niaves of Elridge. He is also survived by several grandchildren and great grandchildren and many nieces and nephews.

A memorial fund has been established at the funeral home in memory of Douglas.
