He was predeceased by: his mother Inah Burkhart McClannahan; and his grandson Alex Cook. He is survived by: his wife Patsy Cook; his sons, Dennis Cook (Lorry), Dale Cook (Lisa) and Jerry Cook (Kim); his grandchildren, Jared, Jon, Josh Cook, Tyler (Chelsey), Leslie Cook, Colton Cook (Abby), Lexie Myler (Noah) and Ashlyn Cook (Creighton); and his great grandchildren, Coraline, Mirabell and Royce Cook.
