He was predeceased by: his parents, Joe Cook and Mary Cook; his sons, Richard Joe and Earl Dean Cook; and his brothers-in-law, Lyle Blake and Norman Wadle.
He is survived by: his daughter Livian Shaver (Ron); his sisters, Karan Wadle and Linda Blake; his nieces and nephews, Anita Stewart (James), Alisha Wise (Dion), Kevin Blake (Amber) and Brandon Blake (Caitlin); and his pet Chase. He is also survived by many great nieces, nephews, relatives, and friends.
