He is survived by: his wife Diana Hoffman of Merrill, IA; his children, Carmen Clark (Scott Galyen) of Des Moines, IA and David Clark (Mary Jo) of Merrill, IA; his grandchildren, Gabriel, Paige and Mikaela; his daughter Sara Quail (Barry) of Westminster, CO; his grandchildren, Lincoln and Kellen; his daughter Shana Herke (Jarret) of Emmetsburg, IA; his grandchildren, Aylee and Emersyn; his son Steven Clark (Melanie) of Granger, IA; his grandchildren, Olivia, Sophie and Eli; his son Jason Clark (Shera) of Ankeny, IA; his grandchildren, Jaelyn and Brinley; and his parents-in-law, Dennis Hoffman (Jean), Sharon Andersen, Marcia Haywood (Paul), Jean Fish (Mel), Myrna Hoffman, Joyce Hoffman, Kevin Hoffman, Craig Hoffman and Lynn Hoffman (Shelly). He is also survived by several nieces and nephews.

Social distancing and wearing a mask are required.
