He was predeceased by: his parents, Clara Francis Cook (Barnes) and Charles Howard Cook; his brothers, Jim and Dale Cook; and his granddaughter Katie Lynn Nielson.
He is survived by: his wife Marilyn Stanislaus of Chicago; his daughters, Carol Nielson (Eric) of Omaha Nebraska, Sharon Bollinger (Wayne) of Naperville Illinois and Linda Fahnlander (Vince) of Plymouth Minnesota; his grandchildren, Brett Bollinger, Brian Bollinger (Marie), Sarah Perretta (Mario), Hannah Petzke (Ryan), John Bollinger, Vanessa Gerhold (Dan), Peter Nielson, Michael, Maria, Nicholas, Anthony, Daniel and Elizabeth Fahnlander; and his brother Mark. He is also survived by his Five Great Grandchildren.
