He is survived by: his daughters, Kara Merritts and Shana Cook (Keith); his grandchildren, Keagan Merritts, Samuel Cook, Tristan Cook, Jayden Cook and Autumn Cook; his siblings, Cindy Krogh and Ed Krogh (Suzanne); his nieces, Jennifer Wang and Jessica Becker (Brett); and his nephew Mike Krogh (Kelly).
In lieu of flowers, memorial donations can be made in Dave's memory to the Iowa chapter of the American Parkinson Disease Association (APDA).
