He is survived by: his wife Patricia Hazen; his children, Tiffini Dunn (Jeff Lariviere) of Terril, IA, Nicholas Hazen (Kayla Crosley) of Colorado Springs, CO, Jacub Hazen (Missy) of Harlan, IA and Curtis Hazen (Kara) of Tennant, IA; his grandchildren, Kyler, Macii, Levi Dunn, Kaden, Kylee, Adalynn, Hadley Lariviere, Ethan, Lydia, Jameson, Tristan Hazen, William, Emma Hazen and Weslynn Hazen; and his sisters, Kathryn LeBreton and Jill Palmer (Phillip).
