He was predeceased by: his parents, John Ludgate and Marie Ludgate (Brooks); his siblings, James Ludgate, Mary Ann Booth and Alice Jean Ludgate; his niece Debra Ludgate; his nephews, Kevin Ludgate and Douglas Ludgate; and his sister-in-law JeanAnne Caldwell.

He is survived by: his wife Patricia; his nephew Dirk Ludgate (Trina); his niece Diana Strait (Ralph); and his siblings-in-law, Sandra Lundquist (Frank) and George Caldwell.

In lieu of flowers, donations in Jack's name may be directed to the Knoxville Fire and Rescue Association.
