He was predeceased by: his grandmother Kate Waln. He is survived by: his parents, Mark Dvorak and Nancy Dvorak (Waln) of Keystone; his wife Maggie Dvorak of Keystone; his sons, Brantley and Hudson; his siblings, Lance Dvorak (Hannah) of Chelsea and Onnah Dvorak of Keystone; his parents-in-law, Steve and Elaine Weirather of Newhall; his grandparents, Dick Waln of Amana, William and Janice Dvorak of Elberon; his grandfather-in-law Marty Weirather of Cedar Rapids; his nephew Everett Dvorak; his aunts and uncles, Gina Kolsto of Marengo, Susan Gessner (John) of Watkins, Kevin Dvorak (Julie) of Omaha, NE, David Dvorak (Mona) of Polk City and Jason Dvorak (Helen) of Madrid; and his parents-in-law, Payton Schirm (Alexa) of Vinton, Randi Decap (Drew) of Bettendorf, Philip Weirather (Britany) of Cedar Rapids and Ethan Weirather of Newhall.
