He was predeceased by: his parents, John Daniel'Jack'Boothby and Bernetta Hass. He is survived by: his wife Rhonda Cooper Boothby; his children, Conrad Boothby (Katy) of Cotopaxi, CO, Nicholas Boothby (Heather) of Cedar Rapids, IA, Jacob Boothby of Goose Lake, IA, Anne Boothby of Clinton, IA and Benjamin Boothby (Tiffany) of Toledo, OH.; and his siblings, John Boothby (Judy) of Miles, IA and Arlee Beckeride (Pat). He is also survived by nine grandchildren, one great-grandchild.
