He was predeceased by: his parents, Fred Zylstra and Gertrude Zylstra (Addink). He is survived by: his wife Karen Faye Boone; his sons, Scott Zylstra (Kelli) of Rock Valley, Timothy Zylstra (Ronna) of Omaha and Matthew Zylstra (Natalie) of Fairfax, Virginia; his grandchildren, Christopher (Megan), Michael, Nicholas, Taylor, Jenna, Keegan and Logan; his great grandchild Mason; and his brother Doug Zylstra (Fran) of Loveland, Colorado..

In lieu of flowers, the family prefers memorials be given to Sanford Hospice, Sheldon, Iowa or First Reformed Church of Inwood.
