He is survived by: his children, Matthew Smith of Hazleton and Jessi Smith of Denver; his fiancée Renee Bockholt of Hazleton; his stepsons, Taylor Simmons of Hazleton and Alan Bockholt of Hazleton; and his grandchildren, Kaylynne Smith, Rosa Smith, Ronan Smith and Avah Simmons.
