He was predeceased by: his parents, Donald F. and Lorraine Geneva Smith (Harlan); his brothers, Robert Smith and Samuel Smith; and his daughter Cynthia Smith.
He is survived by: his wife Teresa Smith of Griswold; his children, Brad Smith of Knoxville, Kristal Fulk (Jimmy) of Atlantic, Matthew Meyerhoff (Jacquelyn) of Audubon and Jordan Bazer (Samantha) of Griswold; his grandchildren, Jacob Fulk, Bentley Bazer and Aurora Bazer; and his siblings, Marilyn "May" Land (John) of Afton, Gary Smith of Carbon and Tom Smith of Carbon.
