He was predeceased by: his father C. Kevin Polk; his sister Kellie Renee Polk; and his grandparents, Merris C., Anne B. Gee, Charles H. and Erma Faye Polk.

He is survived by: his mother Jill R. Polk (Gee) of Glenwood.

He lost his battle with COVID-19 on January 20. 2021.
