He is survived by: his parents, Michael Todd and Rondi Leigh Leedom (Ness) of Gilbert; his sister Jessica Grebner (Colin) of Fort Riley, KS; his grandparents, Eddie, Marcia Leedom of Des Moines, Paul and Debra Ness of Maxwell; his great-grandparent Harlow Koth of Urbandale; his uncle Tony Ness (Jamie) of Maxwell; his aunt Kristi Braafhart (Jeff) of Pella; and his cousins, Sara, Taylor, Toni, Lucas, Brady, Blake, Jayden and Elijah.
In lieu of flowers, memorial contributions may be made to the family in memory of Jacob for use to be determined at a later date.
