He was predeceased by: his parents, Jesse H. Lewis and Edith M. Lewis (Harris); and his siblings, Dale Lewis, James Lewis, Doris Winters and Donald Lewis.
He is survived by: his wife Mary F. Lewis of Fort Dodge; his children, Larry Lewis (Colleen) of Las Vegas, NV, William "Bill" Lewis (Cindy) of Kenyon, MN and Anastasia "Stacy" Weimer (Jim) of Fort Dodge; his grandchildren, Chelsey Anfinson (Andrew) and Daniel Lewis; his great grandchildren, Drew Anfinson, Naomi Anfinson and Miles Lewis; and his sisters, Bonnie Passow of Fort Dodge and Linda Seehusen (Gordon) of Fort Dodge. He is also survived by his family and friends.
He was awarded the Melvin Jones Fellowship Award for his tremendous contributions to Lions Club International.
