He is survived by: his daughters, Brooklyn, Paige and Evie; his mother Krista Ciucyk (Kenny Coon); his sister Jamie Lara (Nick); his nephews, Triston, Jaydan and Cayd; his aunt Nancy Barlow; his cousins, Sloane, Shane Barlow, Brooke and Bridget Salisbury; his uncle Mike Stone (Sue); his cousins, Nick, Brandi Stone, Werner Ciucyk (Deb), Don and Diane Decker; and also David Salisbury.

In lieu of flowers, a memorial fund has been established for the Barlow girls at Northwest Bank, Fort Dodge.
