He was predeceased by: his parents, Leslie Prochaska and Ludmilla Prochaska (Beros); and his siblings, Leonard Prochaska, Gerald Prochaska and Diana Basta.
He is survived by: his significant other Linda of Estherville; his daughter Tabitha McInnis (David); his grandsons, Sebastian of Worthington, Minnesota. and Samuel McInnis of Worthington, Minnesota.; his stepsons, Cody Phillips of Irving and Gary W. Phillips of Estherville; his good buddy Dan Schrieber; and his siblings, Kay Hummel, Lola Bergman, Shirley Carter, Patricia Prochaska, Robert Prochaska, Donna Grove, Sharon Werner and Gary Prochaska.
