She is survived by: her parents, Richard "Rick" and Lois Czupka (Edmonds); her former husband Tom Pereira; her stepson Michael; and her siblings, Cindy Manson (Todd), Cherie Pugil (Dan), Connie Worrell (Bill), Clint Czupka (Mike Miller), Craig Czupka (Kim), Curt Czupka (Colleen), Cathy Radig (Ross), Cory Czupka (Gina) and Carrie Czupka. She is also survived by many aunts, uncles and cousins.

In lieu of flowers, memorials may be made to a charity of choice you passionately support or the Davenport Handicapped Development Center.
