He is survived by: his former spouses, Tan Phiv and Toya Lee Purk; his wife Tammy Moritz (Tompkins); his children, Chad Hannen, Shane Stephansen, Amber McCalley, Dustin Moritz, Skylasr Moritz, Nathaniel Moritz, Damon Mortiz, Byron Moritz, Destiny Moritz, Erica Levesque and Corey Levesque; and his siblings, Tiffany, Chelsea, Shannon, Brett and Travis.
