He was predeceased by: his parents, Harley Sloan and Verna Sloan (Melvin). He is survived by: his wife Beverly Moffett; his children, Lisa Sloan and Kevin Sloan; his daughter-in-law Tiffany; his granddaughters, Alissabeth, Allison, Lauren and Carly Sloan; and his brother Jim Sloan.

In lieu of flowers, memorial contributions in Jerry's honor may be directed to the family to be divided among several organizations that were important to Jerry.
