He was predeceased by: his parents, Frank Dvorak and Bessie Dvorak (Banes); his grandsons, Seth Dvorak and Dylan Dvorak; and his sisters, Louise Dvorak and Joann Toalson.

He is survived by: his wife Janice Vavroch of Elberon; his children, Susan Gessner (John) of Watkins, Mark Dvorak (Nancy) of Keystone, Kevin Dvorak (Julie) of Omaha, NE, David Dvorak (Mona) of Polk City and Jason Dvorak (Helen) of Madrid; his grandchildren, Scott Gessner of Ankeny, Ashlynn Riley (Daniel) of Bondurant, Matt Gessner (Jennie) of Marion, Lance Dvorak (Hannah) of Chelsea, Onnah Dvorak, Morgan, Conner, Grant Dvorak, Alex, Lauren, Alaina and Myla Dvorak of Madrid; his great grandchildren, Brantley, Hudson Dvorak of Newhall and Everett Dvorak of Chelsea; and his brother Don Dvorak of Vinton.
