He was predeceased by: his parents, Vanual O'Coil and Nannie Belle Underwood (Powell); and his daughter Debbie. He is survived by: his wife Mary Ann; his children, Dave Underwood (Kimmie) of Long Grove and Dee Ann Douthart (Steve) of Davenport; his grandchildren, D. J. (Rebekah), Luke, Karah (Lars), Brittany (Joel) and Selah; and his great grandchildren, Felicity, Owen, Canaan, Clay and Addilyn. He is also survived by nieces and nephews.
