She is survived by: her husband Mike; her children, Theresa Simmer, Michael Juarez, Mark Juarez and Vanessa Warren; her brothers, Dennis Lively and Michael Lively; her stepmother Lynn Lively; and her sister-in-law Lily. She is also survived by many aunts, uncles, nieces, nephews, cousins and friends.

Lisa would be happiest knowing she has inspired those she knew to donate their time or resources to a favorite charity in her memory.
