She was predeceased by: her parents, Walter Kirkby and Dorothy Kirkby (Booth); and her brother Stephen Kirkby. She is survived by: her husband Charles "Barry" Polivka of Norbert's Catholic Church; her daughter Kristin Eilers (Kevin); and her grandchildren, Megan and Matthew Eilers.

Memorials in Sandy's honor may be made to the Wounded Warrior Project or Tunnels to Towers Foundation.
