He is survived by: his wife Rhonda Azure (York); his son Josh; his daughter-in-law Ashley Puckett of Davenport; his grandchildren, Jordyn and Riley Puckett; his siblings, Cheryl Johnson of Champaign, Damion Lee of Champaign, John Lee of Rantoul, Diane Foote of Decatur and Adam Devorak of Morton, Illinois; and his father-in-law Thomas York. He is also survived by two great-grandchildren.
