She is survived by: her children, Barry Smith (Nicole), Roxanna Bernauer (Jerry) and Vince Smith (Monica); her grandchildren, A. J., Zach, Daniel, Lucas, Jacob, Lisa, Donald, Evan and Jamey; her great grandchildren, Eira and Eli; and her sisters, Marilyn, Carol, Elaine and Arlene. She is also survived by many nieces and nephews.

Memorials may be made in Hazel's name to the Moline Church of Christ.
