He was predeceased by: his parents, Forrest Blaylock and Cora Blaylock (Beaver); and his siblings, Don, Jim, Irene, Ilene and Gladys.

He is survived by: his wife Devita of Cresco, IA; his children, Ty Blaylock (Teri) of Cresco, IA and Connie Gray (Curt) of South Jordan, UT; his grandchildren, Kyle Blaylock (Noelle) of Bondurant, IA, Kristyn Blaylock of Cedar Falls, IA, Courtney Gray and Cooper Gray of South Jordan, UT; and his sisters, Florence and Jan. He is also survived by many nieces and nephews.
