He was predeceased by: his father Lavern Stork. He is survived by: his mother Maxine Stork (Lewis) of Council Bluffs; his wife Jacque Stork; his children, Butch Madden (Jade) of Council Bluffs, Matthew Stork (Crystal) of Council Bluffs, Michael Stork (Shae) of Council Bluffs and Rachel Coldwell (Jessica) of Council Bluffs; his grandchildren, Taylor, Nolan, Kaden, Cecilia, Ryker, Jazmyne, Carlos and Daniel; his great grandchildren, Cambria and Apollo; and his sisters, Kathy Moore (Pat) of Carter Lake and Debby Campbell of Wichita, KS. He is also survived by nieces and nephews.
Memorial contributions will be directed by the family.
