He was predeceased by: his parents, Nova Lewis and Ival Lewis (Medlin); his son Timothy Lewis; and his siblings, Ronald Lewis, Glen Lewis, Elmira Kain and Elaine Kain.
He is survived by: his wife Nancy Forbes of Columbus Junction; his sons, Roger Lewis of Cincinnati and Bret Kurtz of Columbus Junction; his granddaughter Paige Kurtz (Derrek Movall); his great grandchild Hazel Grace; and his brother Harold Lewis of Unionville, Missouri. He is also survived by many nieces, nephews.
In lieu of flowers, a memorial has been established at the funeral home in memory of Gary.
