He was predeceased by: his parents, Lewis Dvorak and Julia Dvorak (Kuchynka); and his siblings, Ronald Dvorak and Lori Dvorak. He is survived by: his girlfriend JoAnn Schmitt of Cedar Rapids; his sister Becky Swierz of Evanston, IL; his sister-in-law Linda Dvorak of Clutier; his nephews, Tom Dvorak (Dori), Tim Dvorak, Terry Dvorak, Travis Dvorak (Kelley) and Michael Swierz; and his niece Janel Ruzicka (BJ). He is also survived by thirteen great nieces and nephews; and three great-great nieces.
