He is survived by: his children, Annette Tubbs (David) of Centerville, Todd Baze (Jill) of Centerville and Tom Baze of Centerville; his siblings, Fred Baze (Fern), Larry Baze (Bobbie), Gary Baze (Joye) and JoAnn Putnam; his grandchildren, Jeromy Tubbs (Amy), Heather Tubbs (Blayne Wright), Tyler Baze (Heather) and Andrew Baze; and his great grandchildren, Alyssa Tubbs, Kennedy Smothers and Asher Baze.
