She was predeceased by: her parents, Ernie Cotter and Zella Cotter (Crow); her husband Robert "Bob" Cook; her son Bruce Cook; and her sisters, Dorothy Conover and Doris Reynolds.
She is survived by: her children, David Cook (Millie) of Oxford, Iowa, Kenneth Cook (Tina) of Marion, Iowa and Julie Kauffman (Kimber) of Miramar Beach, Florida. She is also survived by four grandchildren; and nine great-grandchildren.
In lieu of flowers donations can be made in Hazel's memory to the Oxford United Methodist Church.
