He was predeceased by: his parents, Donald Lee Hayzlett and Doris Marie Hayzlett (Angel); and his former wife Jean Marie Luckow. He is survived by: his children, Michael Hayzlett of Nashua, IA, Jason Hayzlett of Moose Lake, MN and Samantha Pare (Mike) of South Windsor, CT; his siblings, Richard Hayzlett (Wanda) of Maynard, Charles Hayzlett of Cedar Rapids, Joanie Mills (Everett) of Independence, Bonnie Jean Forney (Gary) and Carol Smith (Scott) of Independence; his daughter Jodie; and his brothers, David, Jerry and Tom Hayzlett.
