He was predeceased by: his parents, Jim Cook and Dena Cook; his brother Mike Cook; and his sister-in-law Laurie Cook. He is survived by: his children, Jessica Sager (Aaron Holstein) of Camanche, Kathryn Bibbs (Tony) of Urbandale, Jon Cook (Rebecca) of Cuba City, WI and Clarissa Scheper (Josh) of Davenport; his grandchildren, Maia, Isabella, Lauryn, Madeline, Ava, Charlie, Austin, Colten, Dustin and Alivia; his sister Cynthia Switzer of Des Moines; his nieces, Dr. Jennifer Lovdahl and Dr. Christine Haynes; and his nephew James Switzer.
