He was predeceased by: his parents, Mary Anne and Kenneth Bouk (Caldararo); his sister Helene Fazzari (Bouk); and his father-in-law Eugene Martin. He is survived by: his wife Lauri Bouk (Martin); his children, Andrew Bouk, Daniel Bouk, Joshua Bouk and Britney Bouk; his mother-in-law Jo Ann Martin; his godson Richard Zindle; his siblings-in-law, Mark Martin, Timothy Martin and Sheri Zindle; and his pet Chester. He is also survived by many nieces & nephews.
In lieu of flowers, memorials are suggested to the Boone Area Humane Society.
