He was predeceased by: his parents, James Melvin and Ethelyne Melvin; and his wife Linda. He is survived by: his siblings, Marilyn Gould (Jim) and Stewart Melvin (Carol); his children, Teresa Adams (Glen), Robert Melvin (Pamela), Tammy Miller (Michael Morris) and Joe Melvin (Kevin Hunt); his grandchildren, Jessica Hootman (Adam), Nicholas Melvin (Mari) and Levi Miller (Emily); and his great grandchildren, Rhett, Remi Hootman, Emery, Andi, Jordy Melvin and Presley Miller.
In lieu of flowers, memorials may be sent to the Bloomfield Christian Church, where Phil was a Deacon and life-long member.
