He was predeceased by: his wife Carol; his parents, Paul and Katherine; and his brothers, Paul and Tom.
He is survived by: his brother Jeff; and his children, Amy, Molly, Holly and Scott. He is also survived by grandparents, aunts, uncles, and cousins.
Instead of flowers, his family suggests donations to either of these organizations in David's memory.
