She was predeceased by: her parents, Leo Formanek and Violet Formanek (Stanek). She is survived by: her husband Roger Prusha of Belle Plaine; her sons, Steven Prusha (Tina) of Amana and Michael Prusha (Connie) of Iowa City; her grandchildren, Garett Prusha, Laura Roling (Matthew), Ashley Prusha and Mikayla Prusha; her great grandchild Ella Roling; and her stepgrandsons, Chuck and Tom Compton. She is also survived by numerous nieces and nephews.
