He was predeceased by: his parents, Lloyd W. Perkins and Virginia E. Winden Perkins; his sons, Bradley Owen, Martin Lester, Robert Harold and Jason "Jay" Matthew; his wife Joyce Ann Agnes; his daughter Nichole Ann; and his siblings, Richard Perkins, Jackie Flannagan and Patricia Smith.

He is survived by: his daughter Joyce; and his sister Mary Perkins of Mesa, AZ. He is also survived by several nieces and nephews and many friends.
