He was predeceased by: his parents, Elmer Cook and Elsie Cook (Ayers); his grandson Jared Cook; his siblings, Betty Glissman, Norma Vollmuth, Talena Wheatley, Ernest Cook, Darrell Cook, Cleo Cook and Robert Cook; and his siblings-in-law, Dorothy Cook, Bob Wheeler, Leonard Glissman, Ferd Vollmuth and John Wheatley.
He is survived by: his wife Donna Blakely of Lewis, IA; his children, Donald II Cook (Angie) of Atlantic, Dick Cook of Atlantic and Danette Van Hofwegen (Paul) of Las Vegas, NV; his grandchildren, Michaela, Matthew Cook of Atlantic, Dalton Cook (Jordan) of Atlantic, Derek Cook (Kennedy) of Atlantic and Mariah Cook of Maryville, MO; his great grandchildren, Cora Cook of Atlantic, Claire Cook of Atlantic, Ayden Vetter of Atlantic and Abel Vetter of Atlantic; his sisters, Velma Wheeler of Red Oak and Marilyn Weber (Don) of Leesburg, FL; and his pet Max.
