He is survived by: his nieces and nephews, Pam Slippy of Ankeny, Kelly Aldrich of Cambridge, Robin Blue of Marshalltown, Bonnie Delaney of Nevada, Craig Mittlestadt of Collins, Theresa Mittlestadt of Missouri, Dave Mittlestadt (Brenda) of Jewell, Cheri Hausner (Steve) of North Liberty and Steve Larson (Linda) of Ames.
In lieu of flowers, memorial contributions may be made to St. Jude Children's Research Hospital in memory of Jerry.
