He was predeceased by: his parents, Glen Hook and Hattie Hook (Kroeger); his former wife Sara Lee; and his siblings, Leonard Hook and Marlys Bloom.
He is survived by: his wife Elizabeth Bornholdt; his children, David Hook (Jill) of Newton, Iowa and Sue Ann Daley (Greg) of Ames, Iowa; his grandchildren, Talbot Hook, Lindsey Hook, Spencer Hook, Megan Schoen (Nate) and Andrea Daley; his great grandchild Kenlee Schoen; and his sister Connie Sash (Roger) of Omaha, Nebraska..
