He was predeceased by: his parents, Hugh Daniel and Pauline Hazelett (Jolly); his wife Susan JoAnn Jones; his brother Hugh Daniel "Dan" Hazelett; his parents-in-law, Russell and Gwen Jones; and his brother-in-law Terry Jones.
He is survived by: his sons, Jared Hazelett (Amanda) of Cedar Rapids, Joel Hazelett (Angela) of Ainsworth and Jon Hazelett (Kelly) of Riverside; his sisters, Maria Hazen (Ken) of Marshalltown, IA and Marcia Boyer (Dan) of Santa Barbara, CA; his grandchildren, Halle, Bryce, Brena, Olivia and Brant; his brother-in-law Craig Jones (Judy) of West Liberty; and his nieces and nephews, Hugh Daniel Hazelett IV, Janette Hazelett, Bart Hazen, Ben Hazen, Brad Jones, Ryan Jones, Cami Jones and Daniel Boyer.
In lieu of flowers, a general memorial has been established.
