He was predeceased by: his parents, Marshall Sullivan and Jewell Sullivan; his siblings, Sonny, Max "Mose" Sullivan and Charlotte Bice; and his daughter Terri Sullivan.
He is survived by: his wife Helen Durham Sullivan; his children, Teresa Womac (Herb) and Terry Sullivan, Jr; his granddaughter Amber Womac; and his sister Shirley Russell (Robert). He is also survived by several nieces and nephews.
